Introduction: Handgrip strength is a health parameter influenced by physical activity. Medical students are a unique population at high risk of sedentary behavior due to academic demands. The aims of this study is to determine the relationship between the level of physical activity and handgrip muscle strength in the class of 2023 female medical students of Universitas Lampung. Methods: This study used an observational analytical design with a cross-sectional approach, employing a total sampling technique on 100 female students from the 2023 cohort at the Faculty of Medicine, Universitas Lampung. The physical activity level was measured using the Physical Activity Level (PAL) questionnaire, and handgrip strength was measured using a dynamometer. Data were analyzed using the Spearman correlation test. Results: Participants (n=100) had a mean age of 19.9 years. The majority had moderate PA levels (65%) and moderate handgrip strength (74%). A very strong, positive, and statistically significant correlation was found between PA level and HGS (r = 0.718; p < 0.001). Discussion: These findings are consistent with the Specific Adaptation to Imposed Demands (SAID) principle. This indicates that the neuromuscular system adapts specifically to repetitive functional demands, such as in sports with dominant hand use, thereby contributing to an increased functional capacity in the trained area. Conclusion: There is a very strong, positive, and significant relationship between physical activity level and handgrip strength among female medical students at the Faculty of Medicine, Universitas Lampung.
