Abstract

Introduction: Inflammatory Bowel Disease (IBD) is a chronic inflammatory disorder of the gastrointestinal tract associated with gut microbiota dysbiosis. Fecal Microbiota Transplantation (FMT) has been developed to restore microbial balance; however, its effectiveness remains variable due to differences in pre-treatment approaches. This systematic review aimed to evaluate the effectiveness and safety of antibiotic administration prior to FMT in patients with IBD. Method: This systematic review was conducted in accordance with the PRISMA 2020 guidelines. Literature searches were performed in PubMed/MEDLINE and ScienceDirect through May 2026. Eligible studies included randomized controlled trials and prospective cohort studies evaluating antibiotic use before FMT in patients with IBD. The primary outcomes were clinical response and clinical remission, while secondary outcomes included endoscopic remission, donor microbiota engraftment, inflammatory biomarkers, and treatment safety. Results: Antibiotic administration prior to FMT may support therapeutic efficacy by reducing dysbiosis and facilitating donor microbiota colonization. Most studies reported improvements in clinical response and remission, microbial diversity, donor engraftment, and gut microbial composition, although the magnitude of benefit varied across studies. Several studies also reported changes in inflammatory and metabolic biomarkers. Conclusion: Antibiotic pre-treatment before FMT may improve therapeutic outcomes in patients with IBD by enhancing clinical response and remission, improving gut microbial composition, and promoting successful donor engraftment. Further studies with standardized protocols are needed to determine the most effective antibiotic regimen, dosage, and treatment duration.
